Job TypeThis is a permanent position.
Key ResponsibilitiesThe successful candidate will be responsible for:
- Developing and implementing health promotion and well-being programmes to improve patient outcomes.
- Managing patients with acute and chronic medical conditions, providing expert advice and guidance.
- Creating and evaluating individualised treatment plans for patients with chronic diseases.
- Identifying and supporting patients at risk of developing long-term conditions.
- Providing advanced nursing care to patients, adhering to clinical guidelines and best practices.
- Collecting and processing pathological specimens, and interpreting test results.
- Collaborating with the clinical team to provide comprehensive patient care.
- Maintaining accurate and up-to-date clinical records.
- Contributing to the development of practice targets and quality improvement initiatives.
- Liaising with external services and agencies to ensure patient support.
We are looking for a highly skilled and experienced Advanced Nurse Practitioner who can make a positive contribution to our team.
