We are seeking a highly skilled and experienced Social Worker to join our Adults Team at Worcestershire County Council. As a Social Worker, you will be responsible for assessing and providing care and support to adults with physical disabilities and mental/physical ill health.
Key Responsibilities- Conduct thorough assessments of adults' needs and develop personalized care plans
- Work collaboratively with health colleagues, statutory partners, and voluntary sector providers to ensure effective care and support
- Manage a caseload of adults with complex needs, providing guidance and support as needed
- Develop and maintain relationships with service users, their families, and carers to ensure their needs are met
- Stay up-to-date with relevant legislation, policies, and procedures, including the Care Act and Mental Capacity Act
- Qualification in Social Work or a related field
- Experience working with adults with physical disabilities and mental/physical ill health
- Strong assessment and planning skills, with the ability to develop and implement effective care plans
- Excellent communication and interpersonal skills, with the ability to work collaboratively with a range of stakeholders
- Ability to work autonomously and as part of a team, with a flexible approach to working hours
