About the Role

$28,200 - $30,500 per annum (dependent on location)

We are seeking an experienced Person-Centered Care Worker to join our team. As a member of our care team, you will be responsible for providing enabling care and promoting independence. Key responsibilities include implementing person-centered support plans, managing risks, and ensuring legal compliance.

Key Responsibilities:

  • Implementing individual outcome and wellbeing plans
  • Developing flexible activity plans to promote autonomy and personal growth
  • SUPPORTING individuals in maintaining healthy lifestyles and managing health needs
  • Facilitating effective communication and maintaining detailed records
  • Overseeing medication processes and supporting self-administration
  • Maintaining a safe and clean living environment and adhering to health and safety protocols
