**Job Title:** Advanced Community Practitioner

We are seeking an experienced and dynamic professional to join our team as an Advanced Community Practitioner. This is a unique opportunity to work within the Liverpool Community Respiratory Team, providing hospital admission avoidance and early supported discharge for patients with respiratory conditions.

The ideal candidate will possess excellent clinical skills, leadership abilities, and experience in managing complex respiratory cases. They will be responsible for delivering high-quality care, treatment plans, advice, and guidance to patients with highly specialist respiratory conditions.

As a key member of the team, you will work closely with other healthcare professionals to ensure seamless patient care and support. Your strong communication and interpersonal skills will enable you to build effective relationships with patients, families, and multidisciplinary teams.

**Key Responsibilities:*

  • Provide expert care and management for patients with complex respiratory conditions.

  • Participate in quality improvement initiatives and contribute to the development of best practices in respiratory care.

**Requirements:*

  • Registered Advanced Practitioner with relevant qualifications and experience.
  • Demonstrated expertise in respiratory care, including assessment, diagnosis, and management of respiratory conditions.
  • Strong leadership and communication skills, with ability to work effectively in a fast-paced environment.
  • Ability to work autonomously and as part of a team, with a focus on delivering high-quality patient care.

**Salary:** £45,000 - £55,000 per annum, depending on experience.
