Job Summary

District Community Nursing Sister/Charge Nurse - Band 6

We are seeking an experienced and enthusiastic nurse to join our team as a District Community Nursing Sister/Charge Nurse. This is a challenging and rewarding role that requires strong leadership and management skills, as well as a passion for community nursing.

About the Role

This role involves providing support and leadership to a team of community nurses, working closely with patients and their families to deliver high-quality care in the community. You will be responsible for managing a complex and diverse caseload, actively working to prevent admission to the acute sector and supporting patients in their preferred place of care.

Key Responsibilities
  • Manage a defined caseload of patients, with continuing responsibility for the assessment of care needs, the development, implementation, and evaluation of care programmers.
  • Lead an effective supportive team, working together to provide clinical advice and support to other staff and patients.
  • Responsible for maintaining detailed, accurate and confidential patient records, adhering to Trust policies.
  • Accountable for the standards of care and for determining the activity of your team.

Requirements

To be successful in this role, you will need:

  • A desire to develop leadership and management skills.
  • A passion for community nursing, that matches our passion to provide excellent healthcare for our patients within their own home.
  • Adult nursing qualification along with NMC registration.
  • Ability to work autonomously and as part of a team.
  • High levels of effective communication, influencing and interpersonal skills.
  • Good organisational skills.
  • Ability to motivate self and teams to positively manage change.
