We are seeking a skilled and enthusiastic professional to join our Jigsaw team as a Clinical Team Member. The successful candidate will be responsible for managing a clinical caseload involving assessment, clinical nursing intervention, and coordination of services for children and young people with multiple and complex disabilities.
Main Responsibilities
- Assess and develop care plans for children and young people with complex needs
- Work in partnership with children, young people, and their families to provide high-quality care and support
- Contribute to the development and implementation of care plans and interventions
- Collaborate with other healthcare professionals to ensure seamless care and support
Person Specification
- Nationally recognised core qualification in nursing, social work, or psychology with post-registration experience working with children or young people with additional needs
- ENB 998/Mentorship for Health & Social Care Practitioners qualification
- Experience of working in a community or group setting
- Post-registration experience working with children or young people with additional needs
