Clinical Nurse Practitioner - Part Time
We are seeking a skilled Clinical Nurse Practitioner to join our team on a part-time basis, working 30 hours per week. The successful candidate will be responsible for coordinating the delivery and development of specialist services within the Trust.

**Job Summary**
The role involves planning and delivering safe, effective and compassionate patient care in relation to dedicated pathways of care. This includes providing specialist clinical advice and support to patients and clinical staff, as well as representing the service nationwide.

**Key Responsibilities**
1. To coordinate the delivery and development of specialist services through evidence-based practice in line with national standards for patient safety and monitoring.
2. To contribute to the organisation of multidisciplinary, patient-centred care, ensuring that national and local standards/targets for the specialist area are met.
3. To provide expert specialist clinical advice and support to patients and clinical staff, either face-to-face or via telephone consultation.
4. To manage/lead practitioner-led clinics in relation to reviewing patients and optimising complex treatment regimes, facilitating hospital discharge and preventing avoidable hospital admissions.

**What You'll Need**
* A degree in Nursing or equivalent qualification
* Registration with the NMC/HCPC
* Experience in Infection Prevention and Control or a related field
* Excellent communication and interpersonal skills
