Job Description:

Clinica Family Health and Wellness seeks a compassionate Health Navigator to connect patients with essential healthcare services.

About the Position:

  • The Health Navigator will act as a liaison and advocate for patients between their care team, specialty care, and/or other relevant staff.
  • This role requires assisting patients in navigating referrals to community resources and specialty care.

Main Responsibilities:

  • Conduct comprehensive assessments to identify patient needs and barriers to managing medical and behavioral health conditions.
  • Collaborate with patients, caregivers, primary care medical providers, and healthcare teams to develop comprehensive care plans.
  • Provide care coordination for Clinica patients to address medical, psych-social, community, cultural/religious, and financial barriers to care.
  • Coordinate with internal and external resources to meet patient needs and work with providers.
  • Act as a communication expert on social services when interacting with other departments.
  • Manage and monitor mental health referrals to Community Mental Health Centers for patients with higher behavioral health needs.
  • Follow up with high-risk patients, particularly those with depression, as indicated by the depression registry's targeted outcome measures.
  • Resolve conflicts professionally and model strong interpersonal communication skills with all staff.

Requirements:

  • BA/BS degree in Health/Human Services or a related field OR at least five years of equivalent experience in coordinating care for patients with high levels of behavioral health and substance use treatment needs.
  • Certified Addiction Specialist (CAS) preferred.
  • Minimum 2 years of experience providing health education and/or patient navigation required, with preference for behavioral health navigation.
  • Experience working in a community health setting or with underserved populations preferred.

Salary Information: The estimated hourly salary range is $22.00 - $27.00, based on candidate experience and internal equity.
