Clinical Care Manager

We are seeking a highly skilled Registered Nurse to take on the role of Clinical Care Manager at Care UK, a leading provider of care services.

Key Responsibilities:
  • Partner with discharge coordinators to ensure seamless patient transitions.
  • Complete comprehensive clinical assessments within 24 hours of referrals to minimize delays in hospital admissions.
  • Evaluate patients' suitability for rehabilitation programs and brief colleagues on their needs.
  • Oversee the implementation of personalized care plans to deliver high standards of clinical support and patient-centered care.
  • Support patients with complex needs and varying dependency levels.
About You:
  • NMC Registered Nurse with experience in writing thorough care plans.
  • Familiarity with clinical assessment tools like Waterlow and MUST.
