Job Title: Family Support Liaison
Job Summary: We are seeking a skilled Family Support Liaison to join our team at Remedy Social Care. The successful candidate will work closely with families to provide early intervention and support to address emerging worries and improve outcomes for children and young people.
Key Responsibilities:
- Coordinate support with multi-agency partners to ensure a whole family approach is taken to address identified needs.
- Undertake early help assessments with families, create support plans, and chair team around the family meetings to review progress and identify additional areas of support.
- Direct work with children and young people to explore their views and worries to ensure plans/interventions are helping to improve their outcomes.
Requirements: The ideal candidate will have extensive experience as a Family Support Worker in a local authority and be able to work effectively with families and multi-agency partners.
