**Job Title:** Family First Case Manager

**Job Summary:**
We are seeking an experienced Family Support Specialist to join our team as a Family First Case Manager. This role will involve working with children, young people, and families to deliver targeted interventions and support. The successful candidate will have a strong understanding of key policy drivers and recent reports addressing early intervention and prevention. The ideal candidate will have experience of case management, including undertaking whole family assessments, and be able to work with children and families to deliver preventative interventions.

**Key Responsibilities:**
• Deliver targeted interventions to families with complex needs
• Act as the lead professional and main contact for the family
• Complete Early Help Assessments and SMART plans with families and review
• Engage with families using all options available to ensure their needs are met
• Organise Team Around the Family Meetings
• Keep and maintain accurate records of work Requirements

**Requirements:**
• Experience of case management, including undertaking whole family assessments
• Knowledge and understanding of key policy drivers and recent reports addressing early intervention and prevention
• Experience of working with children and families to deliver preventative interventions
• Child DBS registered to the update service
