About the Job

We are seeking a full-time Band 5 Community Nurse to join our Integrated Care Team in Leytonstone. As a Community Nurse, you will be responsible for providing high-quality care to patients in their homes and communities. Your role will involve assessing, planning, and delivering care to individuals with complex needs. You will work closely with other healthcare professionals to ensure a comprehensive and patient-centered approach to care.

Key Responsibilities

* Assess and plan care for patients in their homes and communities
* Deliver high-quality care to patients with complex needs
* Work closely with other healthcare professionals to ensure comprehensive care
* Provide education and support to patients and their families
* Participate in clinical audits and service development

Requirements

* Registered Nurse qualification
* Experience of working in the community
* Ability to demonstrate understanding of clinical audits
* Good understanding of Trust Values and clinical governance
