Job Overview

We are seeking a highly skilled and dedicated Care Coordinator to join our team at Pennine Care NHS Foundation Trust. As a Care Coordinator, you will play a vital role in providing high-quality care and support to individuals with mental health needs.

Key Responsibilities:

  • Act as a Care Coordinator, carrying a caseload and working with evidence-based interventions to assess needs, formulate care plans, and implement care plans.
  • Liaise with all agencies and members of the multi-disciplinary team to ensure the needs of service users are met.
  • Carry out clinic duties, such as administering medication and providing support to service users.
  • Supervise Community Support Workers and peers as appropriate.
  • Maintain accurate and up-to-date electronic records.
  • Offer initial mental health and social care assessments.
  • Participate in the CMHT's duty system on a rota basis.
  • Work in a therapeutically informed way within the multi-disciplinary team to address the needs of service users with complex emotional disorders.

Requirements:

  • Registered Mental Health Nurse, Social Worker, or Occupational Therapist with care coordination experience.
  • Experience in a relevant area of mental health practice.
  • Current driving license or supported driver with use of vehicle.
