Job Summary
We are seeking a dedicated Youth Wellbeing and Mental Health Professional to join our team in providing support to young people transitioning from CAMHS to adult services. This is a temporary contract until August 2024 with the possibility of extension depending on funding.

As a member of our team, you will work closely with young people and their families to provide individualized support and guidance. You will also be responsible for developing and implementing comprehensive support plans that meet the unique needs of each individual.

Key Responsibilities
- Develop and implement individualized support plans for young people transitioning from CAMHS to adult services
- Provide coaching and mentoring to young people and their families
- Collaborate with other professionals to ensure seamless care coordination

Requirements
- A degree in a relevant field (e.g., psychology, social work) or equivalent experience
- Strong knowledge of mental health theories and interventions
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
