Health Care Support Worker

This is an exciting opportunity for a full time band 3 support worker to join a busy community mental health team covering South Caerphilly, helping to monitor the mental health and physical health of those under secondary mental health services.
You will be expected to run clinics with the support of qualified staff and visit patients in their home.
A flexible, compassionate patient focused attitude to work is essential for this post.
The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.
