Job Title: Homelessness Support Specialist

Corus Consultancy is seeking a skilled Homelessness Support Specialist to join our team. As an integral part of our support network, you will play a pivotal role in helping clients achieve their goals and improve their overall wellbeing.

About the Role:

The successful candidate will work with clients who are experiencing homelessness, providing them with tailored support to address their specific needs. This includes assessing client risk, developing safety plans, and facilitating access to health and care services.

We are looking for an individual with experience in working with vulnerable populations, particularly those experiencing mental and physical health issues, substance abuse, and safeguarding concerns.

Key Responsibilities:
  • Co-produce strengths-based and outcome-focused support plans with clients
  • Assess client risk and develop risk management strategies
  • Identify and assess physical and mental health needs with clients
  • Support clients to access health and care services
  • Organise and facilitate activities to enhance client quality of life
Requirements:
  • Experience working with mental and physical health & wellbeing, safeguarding, and risk management
  • Understanding of client needs assessment and move-on planning
  • Knowledge of common health issues and barriers to health access for people experiencing homelessness
  • Knowledge of housing pathways and move-on routes for people moving from supported accommodation to independence
