About the RoleWe are seeking an experienced Senior Practitioner to join our Intermediate Care team in Southwark. As a Senior Practitioner, you will provide leadership and professional expertise to ensure the delivery of high-quality services.
Main Duties:
- Provide leadership and professional expertise to ensure the delivery of high-quality services.
- Support the Team Manager with service development, safeguarding, professional supervision, and other managerial tasks.
- Oversee the practice of the multi-disciplinary team, offering effective leadership, supervision, mentoring, and coaching.
- Lead and cover the duty function within both Intermediate Care and Urgent Response teams.
- Triaging incoming referrals and allocating cases within the @Home urgent response team.
- Ensure timely allocation of work, manage waiting lists and risks, and monitor performance to meet statutory responsibilities.
- Prioritise safeguarding, ensuring effective systems and processes are in place for investigating allegations and incidents of abuse.
Requirements:
- Degree or equivalent in social work
- Current Social Work England registration
- Significant frontline experience in Adults Services within the UK
