We are currently looking to recruit a Clinical Pharmacy Specialist to join our forward-thinking team providing medicines knowledge and information to support the safe and effective prescribing of medicines across the Primary Care Networks (PCNs).

The post holder will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage the medicines related workload in GP surgeries.

Key Responsibilities:

  • Providing clinical pharmacy services to support the safe and effective prescribing of medicines across the PCNs.
  • Working in a multi-disciplinary team in a patient-facing role as well as providing support to general practice staff with regards to prescription and medication queries.
  • Supporting medicines optimisation, medicines safety, quality improvement, clinical audits, staff training and managing some aspects of the quality and outcomes framework and enhanced services.

Requirements:

  • Current registration with the General Pharmaceutical Council.
  • Association of Pharmacy Technicians UK APTUK.
  • Understanding of current medicines optimisation issues.
  • Experience in primary care/working in a GP surgery.
  • Experience in providing clinical pharmacy services.
  • Experience in clinical audit/data analysis.
  • Evidence of commitment to continual professional development.
