Care Coordinator for Residential Rehabilitation

This role is part of our 24/7 residential rehabilitation unit, supporting male and female clients through general support, medication administration, and interventions.
About the Role
- Provide oversight, training, and support to night shift team members, ensuring safe delivery of assisted self-administration of medication.
- Facilitate and record monthly night shift team meetings.
- Organize monthly night shift/weekend rotas in consultation with the Service Manager.
- Ensure continuity of service by providing or arranging cover for gaps in the rota.
- Assist in the induction and training of team members, including observations of medication administration.
- Participate in an overnight and weekend on-call rota as part of the response team.
