We are seeking a Care Coordinator to join our team at Thera South West. As a Care Coordinator, you will play a vital role in supporting individuals with learning disabilities to achieve their personal goals and aspirations. You will work closely with our team to develop and implement person-centred plans, ensuring that each individual receives the support they need to live a fulfilling life.
Key Responsibilities:
- Develop and implement person-centred plans in collaboration with individuals, families, and other stakeholders
- Provide support and guidance to individuals with learning disabilities to help them achieve their personal goals and aspirations
- Work closely with our team to ensure that each individual receives high-quality care and support
- Participate in regular team meetings and contribute to the development of our care and support services
