Patient Care Coordinator

Ixworth Surgery is a leading provider of healthcare services, seeking a highly skilled and experienced Patient Care Coordinator to join our team. As a Patient Care Coordinator, you will play a vital role in ensuring the smooth operation of our dispensary, providing exceptional patient care and support to our patients.
About the Role:
- Coordinate patient care activities, including medication management, appointment scheduling, and test results follow-up.
- Provide administrative support to healthcare professionals, preparing patient records and reports.
- Communicate effectively with patients, colleagues, and other stakeholders, responding to patient queries and concerns.
- Maintain accurate records, including patient medication lists and treatment plans.
- Collaborate with other healthcare professionals to provide comprehensive patient care, responding to patient needs and concerns.
Requirements:
- Experience working in a health care setting, preferably in a General Practice environment.
- Ability to work flexibly to meet practice demands and provide cover when needed.
- Strong IT skills, including proficiency in Microsoft Office applications.
- Excellent communication and interpersonal skills, able to work effectively with patients, colleagues, and other stakeholders.
Salary: £24,000 - £27,000 per annum, depending on experience.
