Role Overview

We are seeking a highly skilled Comprehensive Care Specialist to join our team in Wembley. As a key member of our community-based care package, you will be responsible for providing expert nursing support to adult clients and their families.

About the Role
This is an exciting opportunity to work with a well-established provider of complex care packages, specialising in adult complex care. The successful candidate will be based in the Wembley area, where we have secured further packages as part of our growth phase.

Your Key Responsibilities
You will design and deliver tailored care plans specific to each client's needs, effectively managing multidisciplinary teams through strong communication and teamwork. Ongoing monitoring, support, and assessment of clients' physical and physiological needs will be a crucial aspect of your role. Additionally, you will provide nursing interventions, including medication management, and build sophisticated relationships with clients' families. You will also collaborate closely with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke packages of care.

Your Qualifications and Experience
To succeed in this role, you will require:
  • RGN with an active NMC PIN
  • A supportive management style that brings out the best in others
  • Up-to-date experience of tracheostomy care and ventilators within the last 2 years
  • Advanced communication skills, both verbally and written, to liaise effectively with patients, families, and multidisciplinary teams
  • Suitability for Band 5/6 Nurses with evidence of effective leadership and management of HCAs
  • A full UK driving licence is desirable but not essential

What We Offer
In return for your expertise, we offer a competitive salary of up to £45,000 per annum (DOE), flexible working arrangements, including one day per week working from home and four days in the community, Monday to Friday hours, 33 days annual leave, a private pension scheme, use of a company pool car or travel card, business travel expenses, and an excellent Learning and Development programme.
