Job Summary

We are seeking a highly skilled and experienced Rehabilitation Case Manager to join our team at Health Case Management Limited. As a Rehabilitation Case Manager, you will play a critical role in supporting individuals who have suffered life-changing injuries, working closely with them, their families, and healthcare professionals to achieve the best possible recovery outcomes.

Key Responsibilities:

  • Conduct thorough assessments of patients to determine their clinical needs and develop comprehensive rehabilitation plans.
  • Work with a range of stakeholders, including solicitors, insurers, and healthcare professionals, to ensure seamless coordination of care and effective communication.
  • Manage complex caseloads, prioritizing tasks and deadlines to ensure timely reporting and administrative requirements are met.
  • Provide ongoing support and guidance to patients, their families, and carers, empowering them to take control of their rehabilitation journey.
  • Collaborate with multidisciplinary teams to deliver high-quality, patient-centered care, and make informed decisions about treatment and care.

Requirements:

  • UK-recognized qualification in Nursing, Physiotherapy, Occupational Therapy, Clinical Psychology, or Sports Rehabilitation.
  • Active professional board registration and public liability insurance (HCPC, NMC).
  • At least five years of clinical or practitioner experience.
  • Excellent written and communication skills, with the ability to compile comprehensive reports and maintain professional email communication.
  • Ability to assess holistically and compile clinically sound recommendations for intervention.
  • Ability to convince and influence stakeholders by sound clinical judgment and reasoning.
