We are looking for an experienced Occupational Therapist to work with our clients in South Wales who require care and support due to neurological conditions. As a Community Care Coordinator, you will work closely with the multidisciplinary team to develop and implement individualized rehabilitation plans.

Key responsibilities include enabling clients to transition to longer-term community care, supporting those with neurological conditions, collaborating with the team, and monitoring progress. You will also be expected to stay up-to-date with the latest research and best practices in occupational therapy.

The ideal candidate will have excellent communication skills and be able to build strong relationships with clients and colleagues alike. We offer a competitive salary range of £35,000 - £45,000 per annum, depending on experience.
