About the RoleWe are seeking a highly motivated Care Coordinator Team Lead to join our Community Occupational Therapy and Visual Impairment team. As a key member of our team, you will work closely with professionals across housing, health, and social directorates to achieve outstanding outcomes for adults and children in Cheshire West and Chester.
Your primary responsibility will be to lead a team that delivers specialist equipment, adaptations, skills gain, and risk management programs. You will also be instrumental in developing creative solutions to meet the needs of adults and children, ensuring they receive high-quality, person-centered occupational therapy.
About the PositionThis hybrid post offers the opportunity to work from one of our council offices, conduct visits throughout the borough, and work from home. We believe in fostering a dynamic and inclusive work environment that supports personal growth and development.
Key Responsibilities- Work collaboratively with local people to understand their care and support needs, identifying options available to them.
- Lead a team that delivers OT and Visual Impairment specialist interventions, enabling adults and children to live great lives.
- Promote connectivity between the team and support services, community assets, and resources needed to promote enabling great lives and community-led support.
- Engage with the proactive and knowledgeable management team to ensure seamless support.
- Develop and progress in your career through training and development opportunities, including professional registration fees as standard.
- Undergo an enhanced DBS check and Adults and Children Barred List assessment.
- Bring experience working in Social Care and/or Health community settings.
- Demonstrate awareness of multi-disciplinary working and integrated approaches within housing, health, and children directorates.
- Liaise and collaborate effectively with clients, families/carers, other professionals, and agencies to achieve the best outcomes.
- Have a working knowledge of Health and Social Care Policy, Safeguarding, housing, and other legislation affecting practice.
- Show ability to assess and manage complex clinical risk.
- Be a self-motivated individual passionate about making a difference in adult social care.
