About the Role

The NHS South Yorkshire Integrated Care Board is seeking a skilled and experienced Patient Care Navigator Leader to join our team. As a key member of our Continuing Healthcare / Funded Nursing Care Team, you will be responsible for delivering high-quality patient care services across all aspects of care.

Key Responsibilities:
  • Facilitate Continuing Healthcare/Funded Nursing Care assessments and reviews for patients, ensuring timely completion and accurate maintenance of patient records.
  • Collaborate with multidisciplinary teams, healthcare professionals, and social care organizations to provide seamless patient care.
  • Develop and implement effective patient caseload plans, ensuring up-to-date information and efficient communication networks.
  • Co-ordinate assessment processes and write key elements of patient care plans to reflect presenting health needs.
  • Work with relevant health professionals, CHC colleagues, and business support staff to ensure delivery of key targets and products required.
  • Support the publication of regular reports on progress within given caseloads.

Requirements:
  • Registered Nurse qualification (Children, Mental Health, Learning Disability) with post-registration experience in case management or transferable skills.
  • Excellent knowledge and understanding of care needs presented by individuals with various conditions and disabilities.
  • Able to deliver against objectives, achieving quality outcomes, prioritizing workload, and working to tight deadlines.
  • Strong communication and interpersonal skills, with ability to construct and deliver clear ideas and concepts concisely and accurately for diverse audiences.
