About the Role

We are seeking an experienced Advanced Nurse Practitioner to join our dynamic team in Sedgefield North Primary Care Network. As a key member of our multidisciplinary team, you will work closely with GPs, pharmacists, and other healthcare professionals to provide high-quality patient care.

The successful candidate will have a strong background in primary and community nursing, with experience in nurse-led management of minor illnesses, triage, and clinical leadership. You will be responsible for compiling protocols and clinical guidelines, as well as contributing to the development of clinical policy and procedure.

You will be expected to maintain a positive, collaborative working relationship with the practices' multidisciplinary healthcare team and adhere to professional standards.

Key Responsibilities
  • Provide high-quality patient care in a primary care setting
  • Develop and implement clinical guidelines and protocols
  • Contribute to the development of clinical policy and procedure
  • Work collaboratively with the multidisciplinary team to achieve shared goals
  • Participate in training and development opportunities to enhance clinical skills and knowledge
Requirements
  • Registered first-level nurse with MSc or equivalent
  • Mentor teaching qualification
  • Clinical supervision training and experience
  • Extended independent nurse
  • Community nursing specialist qualification
