Job Title: RN Triage Runner

About the Role:

We are seeking a skilled RN Triage Runner to join our team at Community Hospice & Palliative Care. As a RN Triage Runner, you will play a critical role in providing high-quality patient care and ensuring seamless communication between patients, families, and healthcare providers.

Key Responsibilities:

  • Conduct thorough assessments of patients to identify their needs and develop effective care plans.
  • Collaborate with patients, families, and healthcare providers to ensure comprehensive care and address any concerns or questions.
  • Provide education and support to patients and families on disease management, treatment options, and end-of-life care.
  • Communicate effectively with patients, families, and healthcare providers through timely updates, reports, and phone calls.
  • Document all interactions and care provided in the electronic medical record (EMR).
  • Participate in interdisciplinary team meetings to discuss patient care and develop strategies for improvement.

Requirements:

  • Current licensure as a Registered Nurse in the State of Florida.
  • Minimum of two years of experience in acute care, oncology, home health, or hospice settings.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers.
  • Ability to work independently and as part of a team, with a focus on providing high-quality patient care.
