About the Role

We are seeking a dedicated and compassionate Care and Support Worker to join our team at Brandon Trust. As a Care and Support Worker, you will play a vital role in supporting individuals with their daily living needs, promoting their independence, and enhancing their overall quality of life.

Key Responsibilities

  • Empower individuals to live the lives they want to lead by providing person-centered support and promoting their autonomy.
  • Develop and implement daily living skills, enabling individuals to manage their daily routines and activities.
  • Support individuals to access their communities, engage in social activities, and pursue their interests.
  • Contribute to the development and implementation of individual care plans, ensuring that each person's needs are met and their goals are achieved.
  • Communicate effectively with individuals, their families, and external professionals to ensure seamless support and coordination.
  • Maintain accurate records and reports, ensuring that all relevant information is up-to-date and accessible.
