About the Role

We are seeking a highly skilled and experienced Senior Mental Health Practitioner to join our team at Berkshire Healthcare NHS Foundation Trust. As a Senior Mental Health Practitioner, you will be responsible for providing high-quality care and support to patients with complex mental health needs.

Key Responsibilities
  • Formulate, execute, and evaluate care plans for patients with significant complexity within a multidisciplinary team.
  • Identify specific interventions and skills required from the multidisciplinary team to deliver evidence-based care plans and expected health/social outcomes.
  • Devising risk and safety plans with each individual patient and alerting crisis/home treatment team services as required.
  • Support patients to develop Wellness Recovery Action Plans and reduce dependence on Secondary Mental Health Services.
Requirements
  • Competent IT skills and ability to navigate various systems and software packages.
  • Previous knowledge and experience of working in the community and with service users who have complex mental health needs.
  • Experience of formulating risk assessments and working with mental health services.
  • Ability to empathise, be supportive, and sensitive to the needs of others.
