Job Summary

The Norfolk Community Health and Care NHS Trust is seeking an experienced Critical Care Liaison Nurse to join our team. In this role, you will be responsible for assessing patient needs, determining priority of care, and processing referrals accordingly.

You will work closely with the senior clinical team to oversee and respond to remote monitoring systems, ensuring high standards of healthcare delivery to patients. Additionally, you will develop knowledge and skills in SystmOne ledger management, identifying areas of escalation that could impact capacity across the system.

Key Responsibilities:

  • Escalate concerns when appropriate by liaising with clinical teams in the community and the integrated team.
  • Assess, prescribe, and arrange care interventions and assessments for all patients referred to the Single Point of Access.
  • Work within the integrated team to prevent unnecessary hospital admissions utilizing available pathways.

Estimated Salary: £35,000 - £42,000 per annum depending on experience.
