About the Role

We are looking for an experienced Social Worker/Care Coordinator to join our team in Chelmsford, Essex.

The ideal candidate will have a degree or equivalent in social work and current registration with Social Work England.

You will be responsible for assessing, managing, careplanning, supporting, and reviewing care needs in line with policy, procedures, and legislation.

  • Key Objectives:
  • Provide high-quality social work services to adults in need.
  • Develop and implement care plans that meet the individual needs of service users.
  • Work collaboratively with service users, their families, and carers to achieve positive outcomes.
Requirements
  • Significant frontline experience in Adults Services.
  • Current Social Work England registration.
  • Disability support available.

Salary: £42,000 - £47,000 per annum
