About the Role

As a Clinical Pharmacist, you will have the opportunity to work closely with patients, GPs, and other healthcare professionals to improve patient outcomes and make a real difference in the community.

Main Responsibilities
  • Support Primary Care Networks (PCNs) with the delivery of safe, effective systems for medicines optimisation, reducing waste and maximising patient outcomes through structured medication review.
  • Contribute to new ways of working and using the skills that your role can bring to evolving PCN services.
  • Responsible for promoting good practice in line with clinical and operational guidelines and clinically support pharmacy technicians with medicines reconciliation post discharge and other medicines projects.
  • On a day-to-day basis, spend time within PCN hubs and in GP practices to answer clinical queries from patients, local GPs and clinicians, trusts and other services such as care homes.
  • Build strong relationships with community pharmacy and trust pharmacy colleagues.

Person Specification
  • Essential:
    • Vocational Master Degree in Pharmacy plus 1 year pre-registration experience
    • Postgraduate Pharmacy qualification or equivalent vocational experience
    • Post registration clinical experience in a relevant area of practice
    • Significant relevant and ongoing CPD
  • Desirable:
    • Independent Prescriber

    • Understanding of the wider determinates of health, including social, economic and environmental factors and their impact on communities.
    • Good motivational skills and the ability to positively promote change in a complex healthcare environment
    • Excellent written communication skills
    • Produce timely and informative reports
