Primary Care Network Pharmacist

We are seeking a highly motivated and experienced Pharmacist to join our team at Wirral University Teaching Hospitals NHS Trust. As an Integrated Primary Care Network Pharmacist, you will play a key role in providing clinical pharmacy services to our local population.
Key Responsibilities- Support PCNs with the delivery of safe, effective systems for medicines optimisation, reducing waste and maximising patient outcomes through structured medication review.
- Contribute to new ways of working and using the skills that your role can bring to evolving PCN services.
- Promote good practice in line with clinical and operational guidelines and clinically support pharmacy technicians with medicines reconciliation post discharge and other medicines projects.
- Build strong relationships with community pharmacy and trust pharmacy colleagues.

Requirements- Vocational Master Degree in Pharmacy plus 1 year pre-registration experience.
- Postgraduate Pharmacy qualification or equivalent vocational experience.
- Post registration clinical experience in a relevant area of practice.
- Significant relevant and ongoing CPD.
