Carr Gomm is seeking a dedicated and compassionate Support Practitioner to join our Tarbert Care at Home service. As a Support Practitioner, you will play a vital role in supporting individuals to lead a good quality of life in their own homes.
Key Responsibilities
- Support individuals with their daily life routine, as per their support plan, which may include meal preparation, medication administration, and personal care.
- Work collaboratively with individuals to help them sustain, recover, or manage their health, skills, and abilities to lead independent and fulfilling lives.
- Provide support to those approaching the end of their lives in their own homes, where this is their choice.
- Develop and maintain positive relationships with individuals, their families, and other stakeholders to ensure a person-centered approach.
Requirements
- Eligibility to work in the UK or ability to apply for a visa granting permission to work in the UK.
- Ability to work in a fast-paced environment and prioritize tasks effectively.
- Excellent communication and interpersonal skills.
- Ability to maintain confidentiality and handle sensitive information.
