Care Delivery Specialist

Unity Healthcare Recruitment Ltd is seeking a highly skilled Care Delivery Specialist to join our team in Birmingham. As a key member of our community healthcare team, you will be responsible for delivering high-quality nursing care to patients in their own homes or community settings.
About the Role- Develop and implement individualised care plans to meet the unique needs of each patient.
- Provide health education and advice to patients and carers to support self-management and promote independence.
- Work autonomously in patients' homes and community settings, ensuring safe and effective care delivery.
- Manage a daily caseload, prioritising tasks and responding to changes in patients' conditions.
- Registered Nurse (Adult) with current NMC registration.
- Clinical experience in a relevant setting, such as community, primary care, or acute services.
- Ability to work independently and manage a caseload in a community environment.
- Strong clinical assessment and decision-making skills.
- Excellent communication, interpersonal, and organisational skills.
