NHS Herefordshire and Worcestershire Integrated Care Board is seeking a highly motivated and enthusiastic expert in Clinical Continuing Healthcare (CHC) to take on the role of Director of Operations Home Based.
This dynamic and rewarding position involves taking the operational lead for delivering the strategic direction and delivery/performance of the busy Clinical Operational All-Age Continuing Healthcare service across Herefordshire and Worcestershire.
The post holder will work collaboratively with other Heads of CHC Service and Associate Director of Nursing for CHC, as part of an innovative Leadership team which is part of NHS Herefordshire and Worcestershire Integrated Care Board (HWICB).
Key Responsibilities:- Take direct managerial leadership for delivering a high-quality, effective, and efficient CHC service with a clear focus on person-centred care.
- Work with all stakeholders to ensure service planning and developments reflect identified local needs.
- Provide clinical leadership and management of the CHC Operational service, including the CHC process from referral to verification, Fast Tracks, DTA, and pathway 4 discharges.
- Adhere to locally agreed quality, performance, and financial targets and regularly report progress against corporate and operational objectives.
- Lead and manage a large and diverse team, including Clinical Leads and Clinical Team leaders across the Assessment and Fast Track team.
