About the Role

We are seeking a highly motivated and proactive individual to join our Patient Flow team at The Clatterbridge Cancer Centre NHSFT. As a key member of our team, you will play a vital role in ensuring the safe and timely discharge of patients, working closely with our multidisciplinary team to achieve this goal.

Key Responsibilities
  • Actively participate in discharge planning, working closely with patients, relatives, and healthcare professionals to ensure a smooth transition from hospital to home.
  • Communicate effectively with patients, relatives, and colleagues, demonstrating excellent written and verbal communication skills.
  • Respond to telephone enquiries and relay messages accurately, courteously, and competently.
  • Work effectively as part of a team, supporting senior colleagues to implement agreed discharge plans.
  • Attend weekly Length of Stay meetings and contribute to the discussion on patient flow and discharge planning.
  • Be responsible for maintaining accurate and up-to-date bed positions for the Trust, emailing the information as required.
  • Assist the Patient Flow Team Leader with audits, data collection, and review of bed utilization, producing reports to demonstrate results.

Requirements
  • Administrative or clinical experience in a healthcare setting.
