As a Rehabilitation Officer, you will play a vital role in enabling tenants to manage their tenancies and move towards greater independence. Your flexible and individually tailored rehabilitation program will focus on providing comprehensive support to tenants, helping them to recognize and develop existing skills, and address their individual needs and anxieties.

 

Key Responsibilities:

Supporting Tenants:

• Develop comprehensive understanding of initial care needs assessment process and ongoing assessment methods.

• Collaborate with tenants to devise rehabilitation support plans, outlining objectives, interventions, time frames, and outcomes.

• Assist tenants in writing their own support plans and agree on review dates and monitoring mechanisms.

• Develop risk assessments and risk management plans based on tenant needs and risks.

• Encourage tenant independence by recognizing and developing existing skills, considering individual abilities, anxieties, preferences, and health needs.

• Record daily notes demonstrating monitoring and identification of tenant responses to interventions outlined in support plans.

• Identify and maintain a log of tenants' health care needs and address them accordingly.

• Maintain regular communication and good relationships with relatives, advocates, and other professionals supporting tenants, and keep a record of all contact.

• Support tenants in the community and on holidays abroad.

Policies and Legislation:


• Familiarize yourself with and adhere to all Paradise Independent Living policies, standards of conduct, and guidelines for staff.

• Identify and address issues if staff are working outside PIL's policy framework and legal guidance.

• Support PIL quality standards and help provide evidence of how these are being met.

• Undertake relevant vaccinations as required by legislation and/or guidelines for the business.

• Complete relevant documentation accurately and to the required standard when on shift.

• Be available to work shifts on a rota, according to the needs of the tenants, providing support on any day of the week, including bank holidays and sleep-ins.

• Undertake other duties as required by the Care Service Manager to support the service.
