Job Title: Community Staff Nurse Evening Service

We are seeking an experienced nurse to join our team as a Band 5 Community Staff Nurse, working part-time (16 hours/week) in the evening service.

As a Community Staff Nurse, you will work within a Multi-Disciplinary Team in the community, providing care to patients across Tameside. Your responsibilities will include assessing patient care needs, developing personalized care plans, and working closely with other healthcare professionals to deliver high-quality care.
