Role Overview

We are looking for a dedicated Home Treatment Team Practitioner to join our team. As a Community Psychiatric Nurse, you will work in the community, providing mental health support to patients and their families. You will be part of a dynamic team that delivers comprehensive care, working closely with healthcare professionals and service users.

Your primary responsibility will be to assess capacity and obtain informed consent from service users, working within a legal framework where necessary. You will also develop and implement individualized care plans, promoting choice and independence for service users.
