About the Role

We are seeking an experienced Registered Band 6 Practitioner to join our dynamic multi-disciplinary team, providing Community Rehabilitation Services to the Mental Health and Learning Disability Division for the areas of North Wales.

The successful candidate will be responsible for providing intensive community rehabilitation to enable Service Users with Complex Emotional Needs and/or Complex Rehabilitation needs to remain within their local area.

Key Responsibilities

  • Provide mental health assessment and rehabilitation services to service users
  • Work collaboratively with the multi-disciplinary team to develop and implement individualized care plans
  • Conduct risk assessments and develop strategies to mitigate risks
  • Provide education and support to service users and their carers
  • Collaborate with other healthcare professionals to ensure seamless care delivery

Requirements

  • Professional qualification in adult mental health or equivalent
  • Post-qualifying experience in mental health and/or rehabilitation
  • Excellent communication and interpersonal skills
  • Ability to work in a pressurized environment
  • Commitment to raising standards in mental health services
