About the Role

We are seeking an experienced Registered Band 6 Practitioner to join our dynamic multi-disciplinary team, providing Community Rehabilitation Services to the Mental Health and Learning Disability Division for the areas of North Wales.

The successful candidate will be responsible for providing a focus on the repatriation of services users back to their home area, and also providing intensive community rehabilitation to enable Service Users with Complex Emotional Needs and or Complex Rehabilitation needs to remain within their local area.

Key Responsibilities
  • Provide community rehabilitation services to individuals with complex emotional needs and/or complex rehabilitation needs
  • Work collaboratively with the multi-disciplinary team to provide a focus on the repatriation of services users back to their home area
  • Develop and implement individualized rehabilitation plans to promote independence and community integration
  • Conduct assessments and provide interventions to address complex emotional and behavioral needs
  • Collaborate with other healthcare professionals to provide comprehensive care and support
  • Provide education and training to service users, carers, and other healthcare professionals
Requirements
  • Professional qualification in relation to adult mental health or educated to degree level
  • Evidence of continued professional development
  • Demonstrate competency of post-qualifying experience in mental health and/or able to demonstrate substantial experience in the assessment, rehabilitation, and management of service users requiring long-term management within the community
  • Team working skills, good knowledge of mental health assessment and assessment tools, evidence of ability to work within identified risk areas in a positive manner utilizing team support
  • Ability to prioritize and work proactively, effective verbal and non-verbal communication skills, ability to work in a pressurized environment
  • Ability to work with service users/carers as equal contributors to service planning and delivery, ability to see service users and carers as equal partners in the management of their own care and support
  • Ability to supervise and manage junior members of staff
