The Role

We seek an experienced Care Manager to join our dedicated team, driving the evolution of community-based care. As a key figure in this dynamic role, you will oversee a caseload of clients with intricate care requirements, ensuring each individual's needs are thoroughly assessed, understood, and met with compassion and respect.

  • Care Planning and Review: Conducting comprehensive assessments for new clients and regularly reviewing existing care plans to maintain exceptional standards of support, identifying any changes in health or personal circumstances and adapting the care plan accordingly.
  • Supervision and Quality Assurance: Providing guidance, mentorship, and supervision to carers allocated to your clients. Auditing daily documentation to guarantee compliance with regulatory standards and internal quality benchmarks.
  • Safeguarding and Professional Liaison: Addressing more complex issues, including safeguarding referrals. Conducting home visits to evaluate the ongoing suitability of care plans, verifying that your clients' environments remain safe. Offering reassurance to clients and their families that they are receiving the highest standard of care.

Requirements

  • 2-5 years of management experience in social care, healthcare, nursing, or social work settings
  • RMN/RGN registration or a Level 5 Diploma in Health and Social Care (or equivalent) highly desirable
  • Proven experience managing complex caseloads and supervising staff
  • Exceptional written and verbal communication skills, with the ability to liaise effectively with clients, carers, and external professionals
  • Full UK Driving License and access to own vehicle
  • Strong organisational, time management, and problem-solving skills
  • Working knowledge of recruitment best practices, employment regulations, and safeguarding procedures
  • Meticulous attention to detail and the ability to work under pressure in a fast-paced environment
  • Collaborative team player with a flexible, adaptable approach to an ever-changing industry
  • Compassionate, person-centred mindset with a genuine passion for enabling clients to live independently
