Job Summary:

CES Locums is seeking a skilled Nurse Case Manager to join our team. As a Senior Clinical Healthcare Specialist, you will be responsible for managing complex clinical caseloads, conducting assessments, and implementing Personal Health Budgets and ongoing Case Management of Patients funded by Continuing Healthcare (CHC), Mental Health & Learning Disabilities.

Key Responsibilities:

  • Conduct robust, transparent, and multi-agency assessments for NHS Continuing Healthcare/NHS-funded Nursing Care.
  • Assess new applications for client's eligibility against the National Framework for Continuing Healthcare and Funded Nursing Care.
  • Participate in Safeguarding of Vulnerable Adults process related to individuals in Care Homes with Nursing.
  • Undertake robust quality Nursing Assessments and Reviews.
