Care Coordinator for Complex Needs

We are seeking a skilled Care Coordinator to join our team at The Forward Trust. This is a challenging yet rewarding role that requires a compassionate and organized individual to care for drug and alcohol users aged 18+ in supported accommodation hostels across Thanet and Canterbury.
About the RoleThis position involves working with a smaller caseload to offer enhanced support and flexibility for those with high complexity and barriers to engagement. As a Care Coordinator, you will utilize trauma-informed approaches and coordinate care with housing support workers and primary care services.
Main Responsibilities- Carry out comprehensive assessments and risk management plans to ensure the well-being of clients
- Deliver structured interventions and facilitate groups to motivate clients towards recovery
- Utilize motivational interviewing techniques to encourage positive behavior change
- Work closely with other support services to provide seamless care coordination
- Experience in comprehensive assessments, risk management, and SMART care planning
- Ability to deliver structured interventions and facilitate groups
- Strong understanding of motivational interviewing techniques
- Excellent IT skills and knowledge of continuity of care and community services
The Forward Trust offers a competitive salary of £28,000 - £32,000 per annum, plus a contributory pension scheme, annual leave, and employer-matched pension contributions.
