About the Role

We are seeking an experienced and compassionate Community Nurse Specialist to join our vibrant Adult Community Nursing Service. This is a full-time permanent post that offers a challenging and rewarding opportunity to make a difference in the lives of patients.

The successful candidate will have a strong background in community nursing and experience working in multidisciplinary teams. They will be responsible for delivering high-quality care to patients in their own homes and community settings, assessing, planning, implementing, and evaluating care needs under the supervision of Clinical Nurse Specialists, Practitioners, and District Nurses.

This role requires strong clinical skills, including the ability to work autonomously and use your own initiative. You will also need to demonstrate a commitment to promoting health education and independence among patients.
