Job Description

The Care and Wellbeing Coordinator will be responsible for delivering high-quality support services to individuals with mental health needs. This will involve assessing referrals, formulating and reviewing support plans, and collaborating with external agencies to ensure seamless care delivery.

Key Skills and Qualifications
  • Knowledge and Experience: Good understanding of mental health needs, welfare benefits, and housing options. Previous experience in a similar role, working with individuals with mental health needs.
  • Soft Skills: Excellent communication and interpersonal skills, ability to work flexibly, and maintain confidentiality.
  • Education and Training: Relevant qualifications in social work, healthcare, or a related field. Commitment to ongoing professional development and training.
Compensation and Benefits
  • Salary: £28,000 - £32,000 per annum, depending on experience.
