Youth Transition Advisor

We are recruiting a skilled Youth Transition Advisor to work closely with young people transitioning out of care.
The estimated salary for this role is £16.01 per hour, based on a 36-hour working week.
Job Description:- Support young people in establishing face-to-face contact and engaging them in the service.
- Conduct comprehensive support needs analyses with young people and develop tailored plans to achieve their future goals.
- Provide expert guidance to help young people access positive activities, education, training, and employment opportunities.
- Collaborate with statutory agencies and departments to ensure compliance with guidelines and procedures.
- Undertake direct work with young people and key stakeholders to build trusting relationships.
- Foster empowerment among young people by enhancing their self-esteem, confidence, knowledge, and resilience.
- Respond effectively to child protection and adult at risk concerns, adhering to safeguarding policies and procedures.
- Establish connections with other agencies on behalf of young people.
- Develop and maintain strong relationships with young people, linking them to suitable services and support.
- Support young people in accessing housing, health, education, training, and employment opportunities.
- Raise awareness about safe care and protection, CSE, and associated risks.
- Twice weekly payroll.
