Responsibilities:
  1. To deliver skilled nursing care to patients in community settings, using evidence-based practices.
  2. To conduct comprehensive assessments of patients' health needs, develop care plans, and coordinate their care requirements.
  3. To collaborate with the multidisciplinary team to ensure seamless care delivery.

We offer a salary range of approximately £35,000 - £50,000 per annum, commensurate with experience.
